Please thoroughly complete the enclosed loan application.

Return it, and all the forms to me. It is not necessary to complete all the forms, but...

Be sure to sign and return all of them.

Provide documentation, (copies), including but not limited to the following:

• Your most recent paycheck stub(s)
• Your last two (2) years complete Federal Income Tax returns, both personal 1040's and corporate 1120's (if applicable), signed.
• Your last two (2) years W-2's (and/or 1099's).
• Year-to-date Profit and Loss Balance Sheet (if self employed):
• Leases and/or rental agreement for any rental properties not on your personal 1040 tax returns.
• Your last three (3) months bank statements (checking, savings, money market, stock and bonds, etc..) to verify liquid reserves.

It is necessary that a fee be charged to cover the cost of an appraisal and credit report. Please remit your check payable to the Appraiser (to be advised) in the amount of $400.00 and fee of $63.10 per credit report payable to Credit on Line.
